Surgery Check-In/Collect History

Surgery/Dental Patient Check-In


When a surgery/dental patient arrives, a receptionist will guide the client to a room, update the patient's status to "Admit for Surgery", send the patient to Smartflow, and print a cage card. Similar to our current workflow, the cage label will be placed in the bin associated with the client's room and the receptionist will write the room number on the surgery tracking sheet.


Next an LVT will take a patient's history, record the history in Ezyvet, print an "Anesthesia Check-in Sheet", and change the patient's status to "In Hospital."


(Green) The Appointment

  • This is Rover's COHAT schedule with Dr. Corey

  • Hovering over this appointment will show details regarding Rover, his owner, and the appointment itself

  • Double clicking the appointment will open Rover's Clinical Record (Medical Note)

  • Right clicking the appointment will reveal the options shown above


(Orange) Patient Status

  • Hovering over this will reveal numerous status options

  • All surgical/dental statuses are indicated with a Heart

  • When a patient arrives, their status should be updated to "Admit for Surgery"

    • This will immediately send the patient to Smartflow (as indicated by the small smartflow icon)

    • ALL anesthesia patients (canine AND feline) should be set as "Admit for Surgery" one arrival

      • This is important for Smartflow check-in

      • Cat room icons should only be used for non-anesthesia appointments


(Purple) Print Label

  • Hovering over this will reveal a "Surgery/Drop Off Label"

  • Select this to print a cage card


Collecting History


In the exam room, double click the appointment to open Rover's Clinical Record (Medical Note)


(Purple) Add History

  • Click the green "+" to add the patient's history


(Yellow) History Templates

  • Either click the magnifying glass or begin typing to find a History template relevant to your patient's visit

  • In this example, I've selected "COHAT History"


(Orange) Recording History

  • After selecting a template, you can hit "tab" on your computer to move through the history questions

  • Any stand-alone "#INPUT#" is a blank space used to type

    • When you hit tab "#INPUT#" will be highlighted and you can free type the client's phone number


(Pink) Save History

  • Select "Add" to save the patient's history


(Blue) Patient Weight

  • The "Health Status" section is where a patient's weight can be updated

  • Click the green "+" to add a weight


Print the Anesthesia Check-In Sheet


After history has been collected, return to the Calendar by selecting the "Dashboard" tab. Right click the patient's appointment to see the options shown above.


(Yellow) Print Form

  • Hover over "Print Form" to see several options


(Orange) Anesthesia Check-In Sheet

  • Click the Anesthesia Check-In Sheet and select "print"

  • The history you collected will be automatically added to the patient's anesthesia sheet!


Once the patient is placed in a cage, change the patient's status to "In Hospital"
